PET REGISTRATION

Please fill out the information below to register your pet with Unique Apartment Homes. All information must be filled out and complete to get your pet registered BEFORE the pet has moved into the unit. Tenants must provide a current picture of the pet and a valid rabies vaccine/certificate. The rabies must cover the full duration of your lease. If the rabies expires before your lease ends, you must provide management with the updated vaccine report at or before the time of expiration. After management receives the submission and approves the pet, tenants will receive an addendum through their online portal for the pet listed below.

By submitting this form it does not guarantee your pet will be approved. Management reserves the right to refuse any pet/animal for any reason – breed, species, size, location of the apartment, etc.

• Pet Rent: $50/month
• Pet Deposit: $250 non-refundable

(If you have previously paid for your pet deposit, you will not be charged another upon submitting this form. Tenants are to use this form to fully register their pet with us prior to moving in or updating pet records).
